Program Registration Instructions
Preregistration is required for participation in leisure services programs. Registration can be completed in the following ways:
1. Register Online. An email address is required to set up a family account.
2. Fill out a paper registration form. Fill out the form as completely as possible the first time it is used. For subsequent registrations, only the "Head of Household" information (top line), participant information (chart) and signature at the bottom is needed (and any changes to the other information). Turn in your completed form along with payment in person, through the mail, or by using our after-hours drop box: Village of Howard, 2456 Glendale Avenue, Green Bay, WI 54313.
All registrations are accepted on a first come, first served basis. Enrollment cannot be guaranteed. All programs have size limitations. Credit cards are accepted online and in person at the Village Hall. Checks can be made payable to Village of Howard.
Facility Reservations: All facility reservations require full payment with security deposit to reserve a facility. An online facility calendar is available for reference. Please contact Village Hall directly (434-4640) for curent availability. A paper registration form and payment must be submitted in person at Village Hall to reserve a facility. Dates cannot be "held" without registration and payment. All facility rentals are accepted on a first come, first serve basis.
